About Us

Who We Are

Country Meadow Rehabilitation and Nursing Center is located in the town of Bellville, Ohio, offering a specialized 48-bed facility focused on personalized care and rehabilitation services. We are a Veteran-approved center and we strive to create a warm and inviting home-like atmosphere for our residents amidst the tranquil countryside of Richland County. With the privilege of being one of only two VA approved facilities in the area, we are honored to care for our veterans.

Our commitment to individualized therapy programs ensures that each resident receives tailored care designed to address their unique needs with utmost precision. A nurse manager specialized in wound and ostomy care elevates the quality of our services. In moments of relaxation, residents can enjoy the peaceful front porch views and observe the daily farming activities of the local Amish community.

As one of the smaller skilled nursing facilities in Richland County, we prioritize continuity of care and foster a close-knit environment where residents and staff share a strong sense of familiarity. Our experienced therapists offer physical, occupational, and speech therapy, along with the specialized VitalStim® therapy program, all geared towards enhancing functionality and restoring independence.

At Country Meadow, we strive to provide a healing environment that feels like home. We encourage loved ones to bring personal items, making residents’ rooms inviting and personalized. Our dedicated and long-serving staff, many with 17 years of experience, ensure residents are met with familiar, friendly faces every day. The picturesque country setting, complemented by home-cooked meals, further contributes to the contentment of our residents.

We extend a warm welcome to visitors, offering guided tours of our newly upgraded facility, including the therapy gym. We eagerly look forward to discussing the unique healthcare needs of potential residents and their families. Thank you for expressing interest in Country Meadow Rehabilitation and Nursing Center. We take immense pride in being a Veteran approved skilled nursing and rehabilitation facility, devoted to serving those who served our country!
